The catch is the minimum necessary standard: a reminder may confirm that an appointment exists, but it may not reveal why the appointment exists. A text sitting on a lock screen or a voicemail played on speaker can be seen or heard by anyone near the phone.
Safe to include
- Patient first name (or first name + last initial)
- Practice name and general location
- Appointment date and time
- Provider name — unless the name itself reveals a specialty
- Generic prep instructions (“arrive 15 minutes early”)
- Confirm / reschedule instructions and a callback number
- Opt-out language (“Reply STOP to end reminders”)
Never include
- Reason for the visit or procedure name
- Test results — or even “your results are in”
- Diagnoses, conditions, or treatment details
- Medication names or prescription details

Voicemail and live calls have their own limits

What changed for appointment reminders in 2026?
Nothing moved on the HIPAA side — the minimum necessary standard has governed reminder content since 2003, and the 2025 Security Rule proposal is still pending. The action is all TCPA. The FCC's opt-out keyword rule took effect in April 2025, and on January 6, 2026 the agency pushed the organization-wide “revoke-all” rule out a second time, to January 31, 2027, while it reconsiders the rule entirely.
